Request Medical Records

Patients may inspect, obtain copies of or have copies of their medical records sent to another health care provider or individual.

Patients can use our online tool to request a copy of your medical records.

Methodist Health System hospitals and clinics are required under state and federal laws to obtain a complete and properly executed release of medical information form before they may provide a copy of a patient’s records to anyone. In the event the patient is unable to execute the authorization, the patient’s legal representative may authorize the release of information.

In addition to utilizing the Methodist My Care portal or our online request tool, you can obtain medical records by downloading and completing the medical records request form(s) below:

When completing the authorization form:

  • Indicate the dates of treatment for which you are requesting.
  • Indicate the type of records needed by checking the appropriate boxes.
  • When checking Entire Medical Record, sensitive information, such as mental health records and/or substance use disorder records, will not be released without checking these additional boxes.

If the patient is a minor – 17 years or younger in Iowa or 18 or younger in Nebraska – the authorization must be signed by a parent or their legally qualified representative. Proof of legal representation will be required.

For Providers Requesting Patient Records

Providers may obtain copies of their patients' medical records and have them sent via fax or secure email to their location for continuation of care. Most requests are reviewed within 72 hours. If a request involves more than one year's worth of information or numerous encounters, it may take longer to process.

    We encourage contactless access to records, but in-person requests can be made Monday – Friday 8 a.m. – 4:30 p.m. at Methodist Hospital, Methodist Fremont Health, Methodist Jennie Edmundson Hospital and Methodist Women’s Hospital.

    Picking up your requested medical records

    You may pick up your records at a designated location if necessary. You will be notified when your records are ready for pickup. Photo ID will be required.

    Please refrain from visiting our facilities for non-medical reasons if you are exhibiting symptoms of illness.

    Someone else is picking up your requested medical records

    If an individual other than the patient is picking up requested medical records, then that individual will need an original signed authorization letter from the patient stating that he or she has permission to pick up the records and a photo ID.

    We provide records to our patients or other medical facilities at no charge.

    Your medical records contain reports that interpret radiology images. Contact the Imaging Department (Radiology) or Cardiac Center to request a copy of the actual images at (402) 354-7908 for Council Bluffs and Omaha hospitals or (402) 727-3731 for Methodist Fremont Health.

    To request the ability to securely connect your health record to the health management apps you may regularly use (i.e. fitness trackers, dietary apps, etc.), please complete the form on our Connect Your Health Management Apps page.

    The Business Office at Nebraska Methodist Health System maintains billing statement information — your medical records do not contain your billing statements. For more information, please visit our billing and insurance page or call us directly at (402) 354-4230.

    To schedule an appointment to inspect your medical records contact the Release of Information Department at (402) 354-4660. A photo ID will be required.
