Methodist Hospital Community Counseling Program
The Methodist Hospital Community Counseling Program helps youth, families and individuals who might otherwise not have access to mental health services. Confidential counseling is provided by experienced, licensed, masters-level mental health practitioners in select Omaha Public School buildings and community locations. These convenient settings allow counselors to assess, help and refer clients as needed to address their unique mental and behavior health challenges.

To submit a physical form, click on the desired form link, save the form, edit in Adobe Acrobat and print when completed. Return the completed form to the community counselor at the designated location or mail/fax the form to the number or address at the bottom of the packet cover page.
Cost
Acceptable forms of payment include Medicaid, private insurance or self-pay. Foundation assistance and payment plans are available. No one will be denied program services due to inability to pay.
