How to request an estimate
To receive an estimate, complete the Patient Estimate Form. You will need:
- The procedure description. (You can get this information from your physician’s office.)
- Your insurance card.
- Complete the Patient Estimate Form.
The Billing Office will contact you as soon as possible regarding your out-of-pocket costs. If you have questions about completing the form, e-mail firstname.lastname@example.org or call 402-354-2456.
Out-of-pocket patient cost estimates
Most people like to know the price they will pay for goods and services, and at Methodist, we provide our patients with as much upfront pricing information as possible. At a time when your focus should be on care and recovery, we want to relieve any concerns you may have about insurance coverage, out-of-pocket costs, payment options and management of your health care expenses.
Before your scheduled procedure, financial counselors from our business office may call you to answer your questions and explain your probable out-of-pocket cost, payment options and eligibility for financial assistance programs. You will be given the opportunity to pay, provide a partial payment or make payment arrangements. Medically necessary services will not be denied or delayed because of lack of payment.
Remember you have the final responsibility for payment of your bill. If you need help paying your Methodist Hospital bill, Patient Financial Services representatives are available to help you. Make an appointment or walk-in Monday – Friday, 8 am. - 4:30 p.m.
